Kaylah Anne Hawkins Forsyth . . . Kaylah Anne Hawkins died Thursday, July 11, 2013, following an automobile accident. Funeral services will be held at 2:00 p.m., Monday, July 15, 2013, at Lighthouse of Prayer, 2287 Hwy 41 South, Forsyth, GA 31029. Burial will be private. Pastor Jeremiah Hosford and Pastor Red Swinger will officiate. The family will greet friends from 3:00 p.m. until 5:00 p.m., Sunday, July 14, 2013, at Monroe County Memorial Chapel. Kaylah was born September 30, 1999, in Conyers, Georgia. Her father, Khalid Eshakuski preceded her in death. She was a student. Survivors include her mother, Tracy Hawkins of Forsyth, brother, Joshua McCullough of Forsyth, brother, Jacob McCullough of Forsyth, sister, Amanda Eshakuski of Atlanta, brother, Matthew Eshakuski of Marietta, sister, Amy Eshakuski of Atlanta, sister, Farrah Londono of Tampa, Florida, grandparents, Leland and Barbara Hawkins of Forsyth, several aunts, uncles, one niece and one nephew.
